To protect our business from fraud and illegal activity, we require proof of purchase in order to fulfill and ship your order to you. Please fill out the Payment Authorization Form HERE after you've placed your order. 

Only your legal name as shown on your Government ID/ Drivers License should be used when placing orders with Ailani Kiara Vanity. I reserve the right to cancel any orders placed under a nickname or false name.

We have the right to cancel your order if the form hasn't been filled out after 2 business days.
